Application Information

Applications accepted Teachers may request that children living farther from the school pay a special "school visit" to the room, prior to the beginning of school. enrollment is open to children who reach their 3rd, 4th, or 5th birthday by december 31 all children must be toilet trained. the state board of health requires that each child have a certificate of immunization on file. a physician or health professional must sign the health form (supplied at registration) before it is returned on or before the first day of school. to be sure our director is available to show you to a class and answer your questions, call to arrange a visit. (eligible children turn 3 on or before december 31, 2007.) (turning 4 from october – december of 2007). registration card - to be completed at registration. proof of identity - for any child not previously registered. this may be any of the following: the state certified copy of child's birth certificate, birth registration card, or passport call the division of vital records in richmond for virginia birth certificates - 804-662-6200 health form -to be completed and signed by a physician, due the first day of school. "other" forms - several forms will be included with the letter you'll be receiving in july: parent resource (a chance for you to volunteer your time and talents in your child's classroom) parental consent (emergency information due the first day of school) developmental history (information about your child's likes & dislikes, habits & disposition, and experiences prior to school) payment contract (summary statement of fees listed above) permission to administer medication - if your child requires medication during the course of the day, complete this form and give verbal instructions to school personnel.

Acceptance criteria: St. Matthew's offers its services to the community without regard to race, gender, creed, or national origin. When available spaces are filled, children's names are entered on a waiting list
