Families who are interested in attending faith christian academy are encouraged to contact the school at (972) 562-5323 and schedule an appointment and a tour. the first step in the admissions process is to submit an enrollment form to the school office. additional admissions steps may include standardized testing, a student campus visit, and a parent/student interview.

• health form signed by a health care professional. each highlighted area is required!

• signed parent acknowledgment form from fca handbook
• paperwork on any school testing or independent evaluations for learning, emotional, or
behavioral issues if applicable.
