Application Information
| Applications accepted | To receive enrollment information, contact the early childhood services director, debi schwid, at (414) 933-6161 parents or guardians must register in person with the director and the business office. 1. complete both family and child information forms, providing all requested information. 2. sign a waiver giving permission for the child to take part in activities, including field trips away from the center. 3. give consent for emergency treatment in the event that the parent/guardian cannot be reached immediately. 4. provide emergency and pick-up information. 5. child health report must be signed by child's doctor, and daycare immunization report must be filled out and signed by parent or immunization dates attached. meet health requirements as required by state licensing. (see health policy, p. 13) 6. complete the family size/income statement required by the child care food program. 7. complete the child intake form for children under two years of age. 8. complete the infant formula form if the child is under one year of age. 9. complete the bus transportation form if the child is transported by bus. 10. complete the school-age agreement if the child is attending another school for part of the day. 11. complete nh enrollment and fee agreement forms, as well as photo authorization. 12. receive and go through the parent handbook and sign the form at the back of the book, acknowledging receipt of this handbook and willingness to abide by the contents therein. 13. go through the operational requirements chapter of the hfs licensing rules |
