Applications accepted Attach a copy of your child's birth certificate, current immunizations record, each family member's social security card, and a copy of your 1040 income tax return for the most recent year to the application. the household members listed on the income tax form must include the child being considered for enrollment and the number of total household members on your income tax form must match the number of household members you report on the application. if you would like to visit one of our centers, please schedule an appointment through head start of greater dallas' external affairs director, rob massonneau. he can be reached at 972.283.6472 or by email at rmassonneau@hsgd.org
