| Applications accepted | All forms must be updated at least every 6 months, except for the child health assessment which is updated annually. the forms are as follows: * computerized registration form * signed and dated tuition & agreement contract form * child health assessment form pre-school and pre-kindergarten are ages 37 months-6 years & potty trained. |
