| Applications accepted | 1. child's birth certificate and immunization record. 2. social security numbers for all family members. 3. proof of residence within the independence school district, such as: new driver's license,new voter registration card, rental agreement/contract, or utility bill, etc. 4. if applying for head start: income verification: 2 current pay stubs, tanf award letter, unemployment verification, ssi verification or 1040 tax form with w-2s. immunizations must be current in order for your child to remain in school. if your child has any immunizations after enrollment, please let us know. required immunizations are: 1. dpt (diphtheria, pertussis (whooping cough), tetanus) 2. polio (ipv, polio injection vaccine) 3. mmr (measles, mumps, rubella) 4. hib (immunizes against the organism that causes meningitis) 5. hepatitis b (given at any age) 6. varicella (chicken pox) |

Today's “Preschool’s Out” Activity
Trace around your child's foot, with shoe on, on a piece of white construction paper or card stock. Have child cut out the shoe print and add a spooky face. Glue it to a popsicle stick and you have a ghost stick puppet!
