| Applications accepted | A completed application form along with a non-refundable application fee, for each child seeking admission. a copy of the student's birth certificate must be included. a completed pastor's recommendation form. please give the form to your pastor to be completed and sent to us. a completed parent testimonies form. copies of report cards, standardized test scores, and any other information relating to your child's academic experience. a completed \"request for student information\" (included in this packet). please give this form to your child's present/previous school to be completed and sent to us. a signed association membership statement. when all of the above items have been received, an admissions interview with the admissions committee will be scheduled. |
