| Applications accepted | We ask that your child’s immunization record (form 3231 which can be obtained from your pediatrician) be presented with enrollment forms. at the latest, it is due within 30 days of enrollment. after 30 days, we cannot continue service. this is a requirement from the georgia health department and the department of human resources. |
