Application Information

Applications accepted It serves sixteen (16) children ranging in age from 2.9 to 5 years old. children must be 2.9 - 4 years of age and reside in the town of plainfield which includes the villages of plainfield, moosup, central village and wauregan. parents must be working or in school. a child under the age of 3 may enroll in the months of september, october, november or december if s/he turns three by december 31st of that year. children must be toilet trained except for those with special needs children may only be admitted after a parent intake interview recent (within one year) health assessment form including immunization records signed by the physician. this is required by the state of connecticut. a birth certificate must be presented for photocopying. any necessary medication forms. readiness program contract form. program registration form. readiness program acknowledgment form. person information form.

Trace around your child's foot, with shoe on, on a piece of white construction paper or card stock. Have child cut out the shoe print and add a spooky face. Glue it to a popsicle stick and you have a ghost stick puppet!
