Acceptance criteria: Children must be the appropriate age by July 1st to meet enrollment age requirements. Emergency Contact Form Health Form Important! Have your child's physician complete the Health Form and attach a copy of your child's immunization record to this form. Meet with Mrs. VerHage to tour the facility, review the King's Kids Ministries handbook and turn in your child's Health Report. Mrs. VerHage will contact you to schedule an appointment as she begins to process registrations. The first week's tuition payment will be due at this appointment
