| Applications accepted | In order to secure your child's enrollment, the following items must be completed. pay the registration fee, have an interview with the direct, and you must complete and sign the application and parent/school agreement forms consisting of: 1. admission agreement 2. completed physician's report w/ tb test results 3. copy of up to date immunization record 4. pre-admission health history 5. consent for emergency medical treatment 6. identification and emergency information 7. read and sign notice of parent's rights 8. read and sign personal rights 9. read and sign child abuse prevention 10. read parent handbook and acknowledge by signing 11. read and sign all future required forms. |
