Applications accepted A child must be at least two years old to enroll. enrollment forms include an application form, a tuition agreement, a physician's report, a health history, a signed parents' rights form, a signed personal rights form, emergency information forms, and a medical consent form. immunizations must be up to date class for children who are 3 by dec.1s classes for children who are 4 years old by dec. 1st.

Trace around your child's foot, with shoe on, on a piece of white construction paper or card stock. Have child cut out the shoe print and add a spooky face. Glue it to a popsicle stick and you have a ghost stick puppet!
