Applications accepted A child must be four years of age on or before september 30, 2007.in determining acceptance, the school will review the academic, discipline, and attendance records of students transferring from another school. certificate of baptism official state birth certificate student's social security card, immunization record: this card should be made available to you from your doctor or from the ebr parish health unit. a single dose of hib vaccine and a single dose of mmr vaccine is required from all children entering pre-kindergarten with second dose of mmr at or after age 4.
