Applications accepted To begin the application process, please contact the office at 773-714-0646 to schedule an informational visit and tour. a medical form and birth certificate must be on file with the school before your student may start school. classroom visits for prospective students age 2-5 are arranged by appointment from 9:00-9:45 a.m. on a regular school day. we will then request student records from the previous school or day care center
