| Applications accepted | 1. contact christa jackson, director of admissions for an application packet. 2. arrange for a tour of our campus (preferably with your child.) 3. submit completed application form, parent questionnaire, records release request (all available for download below), and a current photo of your child. 4. make an appointment for your child's entrance examination. a $10 exam fee is due at the time of testing. 5. for catholic students only: send a copy of your child's baptismal and first eucharist certificate (if applicable) and fill out the parish verification form. 5. upon your child's acceptance to st. mary's, submit a $250.00 registration fee ($350.00 for middle school). students must be four years of age by september 1 to be enrolled in pre-kindergarten. birth certificate • immunization record (must be on the ga department of human resources certificate of immunization form 3231). prior to the first day of school, students entering kindergarten must have: 3 doses of the hepatitis b. vaccine mmr vaccines varicella (chicken pox) vaccine prior to the first day of school, students entering the 6th grade must have: mmr booster varicella (chicken pox) vaccine georgia state law requires the booster and certification – form 3189 or 3231. • certificate of eye, ear, and dental examinations. this must be obtained from the ga department of human resources or family physician. • custody papers, if applicable • catholic students should provide baptismal certificates, and first communion certificates, if applicable. |

| Acceptance criteria: | Applications will continue to be accepted as long as there is space in the class, but because there is limited space in many classes, it is wise to turn in the application as soon as possible. Siblings of currently enrolled students, • Students transferring from another Catholic School (out of town), • Students from regional parishes, active status, • Students from Regional parishes, non-active status, • Students of another religious affiliation. St. Mary's Catholic School shall admit students of any race, color, gender, national and ethnic origin to all the rights, privileges, programs, and activities generally accorded or made available to students at the corporation's school. St. Mary's Catholic School shall not discriminate on the basis of race, color, gender, national and ethnic origin in the administration of its educational policies, admissions policies, scholarship and loan programs, and athletic and other school-administered programs |

Today's “Preschool’s Out” Activity
Trace around your child's foot, with shoe on, on a piece of white construction paper or card stock. Have child cut out the shoe print and add a spooky face. Glue it to a popsicle stick and you have a ghost stick puppet!
