Application Information

Applications accepted Education and the child must be between the ages of 3 and 4. if you call our office at (626) 854-3449, one of our enrollment specialists can tell you if your child is eligible for our program. in order to enroll your child, you will need to bring: copy of your child's birth certificate immunization records current tb test for you and your child (done within one year) your child's physical completed within one year 2 consecutive paycheck stubs within the past 30 days to show proof of income.
Acceptance criteria: Admission to our program is based upon the following priorities: First Priority is given to children receiving protective services. Second Priority is given to eligible four year olds. Third Priority is given to eligible three year olds

Trace around your child's foot, with shoe on, on a piece of white construction paper or card stock. Have child cut out the shoe print and add a spooky face. Glue it to a popsicle stick and you have a ghost stick puppet!
