| Applications accepted | Prospective parents/guardians are encouraged to visit our school to see our exciting and enriching program for the toddler program children must be 18 months old. for the 2 year-old program, all children must be two years old by december 1, 2007 for the 3-year-old program, all children must be three years old by december 1, 2007. all children must be fully potty trained. for the pre-k program, all children must be four years old by december 1, 2007. all children must be fully potty trained. an appointment will be made to interview and assess children entering the 3-year-old program and the pre-kindergarten program. carden west school internal forms ? state licensing forms: o identification and emergency information o child's preadmission health history o consent for emergency treatment form o personal rights form o physician's report o parent's rights ? birth certificate ? immunization record |
