| Applications accepted | Children enrolled in these classes must meet the age requirements for their class. children enrolled in preschool must be the required age for their class by september 1. (must be walking) prior to admission, the following forms will need to be completed prior to enrollment: registration form, certificate of immunization, child care contract, tuition rate sheet, permission to administer medicine, sick care exclusion, wac abuse form, discipline policy, health and social record, photo permission slip, consent for treatment of a minor, and a signed copy of parent handbook acknowledgement. |
