Application Information

Applications accepted Enrollment opportunities for school year 2007-2008 are available now. please call dave trochim at 756-2424 for information. previous two years report cards. previous two years standardized testing results. a current transcript for high school applicants that includes all of their high school course work. a current report card for applicants k-8. the registration fee is due when an applicant has been accepted for admission. 3 year olds olds: (by september 1st and potty trained) we accept children that are between 3 years (potty trained) and 5 immunization record: filled out completely and signed at bottom. your child can be exempt from immunizations for medical, personal or religious reasons. you must check box on back of immunization form and sign. medical exemption must be signed by a doctor. emergency consent form: this is an important document in case of injury to your child. please fill out completely with doctor, emergency contact numbers, allergies, & medications your child takes on a regular basis. be sure to sign the consent for reatment/admission by hospital and also sign at the bottom of form. check any medical history on back of form.

Acceptance criteria: Reference letters are requested by Life Christian from references supplied on the application (current teacher, pastor, non-relative adult). Must be in our possession before testing and the interview can take place. Completed applications will be given to the Admissions Committee for approval. An application is considered complete after placement testing is finished, all reference letters and other school information is received, and the student interview has taken place. Life Christian operates on a selective admissions process. Applications are NOT processed on a "first come - first served" basis. As applications become complete, and if all parameters are equal, priority is given first to active Life Center member families and to other siblings of current families. Our Preschool is open to all children without regard to race, creed, color or national origin. WAITING LIST PROCEDURES: Classes are filled on a first come first serve basis according to date of enrollment. If classes are full, the child’s name will be put on a waiting list. Parent/guardian will be notified when an opening occurs
