| Applications accepted | You will need to bring the following information to your appointment: 1. most recent 2-3 paycheck stubs. 2. current shot records for each child being enrolled. 3. well check statement from the doctor for each child being enrolled that states that child is in good health and may attend childcare (you can have this faxed to 806-741-0503.) 4. name, address and telephone number of the child(rens) doctor. 5. name, address and telephone number of at least 2 people, other than yourself who can, and will, pick the child(ren) up from the center if we are unable to reach you. 6. food stamp or tanf number if applicable children must be at least three years old by september 1, 2006, live in the lisd school disctrict and must be eligible by income. parents who qualify for ccs (child care services) or reduced or free school lunch program may be eligible. |

Today's “Preschool’s Out” Activity
Trace around your child's foot, with shoe on, on a piece of white construction paper or card stock. Have child cut out the shoe print and add a spooky face. Glue it to a popsicle stick and you have a ghost stick puppet!
