Application Information

Applications accepted Attend an open house information meeting and tour. out of town applicants who are unable to attend such a meeting will complete the initial meeting process via phone interview completion of records release form completion and submission of all required forms, application, birth certificate and immunization/medical form required documents: an odjfs medical form, provided by village academy, must be completed and signed by a physician and will be kept in the student's file. a yearly physical examination is required by the state of ohio for all preschool children.updated immunization records must also be submitted on a yearly basis. the school policy requires new students to submit a recent (within the past six months) physical examination form and inoculation record prior to enrollment. in addition, one emergency transport form and authorized pick up form must be completed by the parents and placed on file in the office. parents of preschool students are required to submit to the school a certified copy of the student's birth certificate.

Acceptance criteria: One day classroom visitation by applicant for evaluation Teacher recommendations by student's present math and English teachers Village Academy Schools admits students of any race, color, creed, sex, or national origin to all privileges, rights, programs, and activities. Trace around your child's foot, with shoe on, on a piece of white construction paper or card stock. Have child cut out the shoe print and add a spooky face. Glue it to a popsicle stick and you have a ghost stick puppet!
