Application Information
| Applications accepted | Our program begins with children 2 1/2 years of age, and fully "potty-trained". please be sure to set an appointment with your child's physician, to complete and sign the medical statement (attached to the information packet). we must have this form returned to our office within 30 days of the date of admission. you will then need to submit it to our office with a registration fee of $40.00. this will reserve your child's space, while the remaining steps are in process of completion. |

Trace around your child's foot, with shoe on, on a piece of white construction paper or card stock. Have child cut out the shoe print and add a spooky face. Glue it to a popsicle stick and you have a ghost stick puppet!
