| Applications accepted | Child's birth certificate or hospital record proof of income: · last year's income tax return · current ssi printout from social security office · social security disability 1099 form or printout from social security office for all members of the family · current tanf printout from family support division · if you do not have the above, a letter from all employers stating gross wages and beginning and ending dates of employment. parent, attorney of fact, guardian's signature before your child can be enrolled we will need your child's: up-to-date immunization records custody papers, if applicable must be 3-years-old before august 1 and not be school-age (5-years-old before august 1). enrollment for full-day classroom placement is determined by the following criteria. ? parent/guardian has difficulty caring for the child due to a disability/medical condition or any special need identified by the family that justifies full day care. or there is no caregiver present in the home due to parent(s)/guardian(s): ? working. ? going to school (college or vocational classes). ? in job training. ? enrolled in the career assistance program (cap). a statement signed by each parent/guardian stating they have met one of the above requirements will be maintained in the child's file throughout the program year. |
