| Applications accepted | Preschool children atleast age three by september 1st. documentation/paperwork required by the state of minnesota before each child's first day of attendance includes: - proof of immunization. - health history, including information regarding health needs and allergies. - emergency contact information. we must have two people besides the child's parents to contact in case of an emergency. - "permission to administer syrup of ipecac" form. |
