| Applications accepted | Hill child development center reserves the right to dis-enroll any child at any time for any reason. no advance notice is required. enrollment procedures first and last week's tuition signed parent/center agreement up to date immunization record completed emergency information record completed family/child information form a complete health appraisal form signed by a physician is due within 30 days of enrollment. medication policies in order for the staff of hill child development center to administer medication, the following provisions must be made: medication must be stored in the original container medication must be labeled with child's name medication will be stored according to the instructions medication will be returned to the parent or destroyed when no longer needed |
