Applications are accepted: Call the center to check available space for your child. forms include: • background information sheet • child care payroll deduction form • enrollment agreement (financial policies, tuition, sliding scale) • child enrollment and health form • child's medical statement • permission form • visit/pick-up authorization form • time & talents form • care of children with handicapping conditions or health conditions form • request for the administration of medication by child day care personnel form (needed if your child has ongoing medication which must be administered at the center.) • infant information • sleep position waiver |

Today's “Preschool’s Out” Activity
Trace around your child's foot, with shoe on, on a piece of white construction paper or card stock. Have child cut out the shoe print and add a spooky face. Glue it to a popsicle stick and you have a ghost stick puppet!
