Application Process
Applications are accepted: Your child's physical: a completed child health assessment form from your pediatrician, including current immunizations. for children 12 months and older, we also need a copy of your child's lead level result, and either a copy of your child's ppd test results or a letter from your pediatrician stating that your child does not need the ppd skin test. your doctor will be able to make this determination based upon your answers to a questionnaire that is designed to determine whether or not your child is at risk for having been exposed to tuberculosis |

Trace around your child's foot, with shoe on, on a piece of white construction paper or card stock. Have child cut out the shoe print and add a spooky face. Glue it to a popsicle stick and you have a ghost stick puppet!
