Application Process
Applications are accepted: All preschool children must be toilet trained. 1)enrollment application. a) if there should be any change in employment, phone numbers or address, fill out a change of information form (available at school site). b) enrollment forms are updated at the beginning of the school year and the summer. 2) discipline & guidance form. 3) release from liability and agreement to indemnity for medical treatment. this form does not release kids & company from all liability but merely points out that there are some situations in which kids & company has no control over. 4) child and adult food program form 1531 (this form is confidential and is exclusively used for and required by state child nutrition programs). 5) please include on the enrollment form any special needs your child might have. 6) signed code of conduct and parent acknowledgment from the parent handbook. 7) children enrolling in the eec program must give a copy of updated shot records including tb test, birth certificate, social security card, and physical from doctor within one month of start date. 8) payment agreement. 9) optional forms: medication forms. |

Today's “Preschool’s Out” Activity
Trace around your child's foot, with shoe on, on a piece of white construction paper or card stock. Have child cut out the shoe print and add a spooky face. Glue it to a popsicle stick and you have a ghost stick puppet!
