Applications are accepted: We ask all parents to complete the emergency card at time of enrollment. this provides us with names, telephone numbers and an up to date immunization record for each child. we also complete any other required documents and release agreements at this time, including d.e.s. forms if applicable children must be enrolled by their parent or guardian |
