Applications are accepted: Child development form physician and insurance information - on child development form local emergency contacts other than parent- on child development form (state requires 2 contacts other than the parents) driver's license and/or social security number- on child development form signatures --------( ?parent ?medical- on child development form licensing statement signed (state requirement) child and adult food program eligibility application (the child and adult food program form must be complete! even if you are not eligible, enter your child's name and age. write “not eligible” where income is requested and sign and date the form.) the registration material must be complete with necessary payments in order to be enrolled. |

Today's “Preschool’s Out” Activity
Trace around your child's foot, with shoe on, on a piece of white construction paper or card stock. Have child cut out the shoe print and add a spooky face. Glue it to a popsicle stick and you have a ghost stick puppet!
