Application Process
Applications are accepted: Submit a copy of the child's current immunizations and completed registration form before the child can be admitted to the program. the registration form includes the child profile, the parent/guardian statement of understanding, and a notarized copy of the medical authorization form. a recent copy of the child's physical examination must be submitted within 30 days of registration. |
