Applications are accepted: Children who will be 2 years old by december 30, we will offer little learners children who are 3 year old by october 31, smart start will be offered children who are 4 years old by october 31, the preschool will offer preschool plus, a program for 4’s and 5’s. (ask about our program for young four’s to be enrolled in this program, we encourage children to be potty trained. completed registration form * current medical statement (on file on child’s first day, and updated every 13 months) * completed child enrollment and health information/emergency transportation form (both sides) * child profile form * signed policy agreement form * medication form completed by physician (* if applicable)
