Applications are accepted: The following forms are required and need to be completed prior to placement. application signed by parent or guardian. registration fee:* $35.00 (full time & half day) emergency card. fully completed immunization and health card. parent/school contract criteria for admission: all children in attendance must be: toilet trained submit proof of immunizations aged 2 ½ to 5 yrs 11 months tmmp reserves the right to withdraw a child who consistently has extreme difficulties fitting into the program. tmmmp will have a meeting with the parents to discuss the difficulties and the possible solutions. if after a week the difficulties are not improving the child will be withdrawn from the program. as long as there is no danger posed to your child or the other children at the center, tmmp will give one-week notice for the parents to find alternative care.
