Applications are accepted: Meet the age requirement. a completed application for enrollment. enrollment or re-enrollment fee paid. a complete health record and immunizations, which is required by the state of michigan. children entering our childcare or preschool must be toilet-trained prior to admission and meet the age requirement for each class. in order to enroll in this class, a child must be 3 years of age by september 1st and toilet trained prior to entry. in order to enroll in this class a child must be 3 â½ years of age by september 1st and toilet trained prior to entry child must be 4 years of age by september 1st to enter this program health form birth certificate
